The State of California requires all drivers to pass a medical examination before they can drive a vanpool.

Who Can Perform the Exam?

If you prefer, you may make an appointment with your own physician. Be sure to have both the DMV Medical Examination Report form and the wallet-sized Medical Examiner's Certificate (provided by your doctor) filled out and signed by the examining physician.

Yes. The fee for the DMV Medical Examination varies. Call several clinics if you wish to compare prices.


After the Exam

Proof of Medical Certification

You must keep the wallet-sized Medical Examiner's Certificate with your license whenever you drive the van. Keep the 8 1/2" x 11" DMV Medical Examination Report form in your private records. Please do not send the DMV Medical Examination Report form to 511 Carpool. If you should lose the wallet-sized Medical Examiner's Certificate, your DMV Medical Examination Report form can be used as proof of certification.

Renewal

Please remember that the State of California requires that you renew your medical certification every two years.
